Patient Forms

As of May 1st, 2009, the FTC has mandated an Identity Theft Prevention Policy be implemented by all medical practices. To comply with this policy, we MUST make a copy of your valid driver’s license, passport or photo identification card. Please bring one of these documents with you to our office. Additionally, if the address on your ID does not match your home address, you must bring a copy of a utility bill which confirms your home address. We appreciate your compliance and apologize for any inconvenience this may cause you.
